Adult Family Homes provide a living arrangement for individuals age 18 or older to remain in their community. The homes are certified by Wisconsin's Health and Human Services System as meeting specific standards. The Adult Family Home provides minimum supervision and assistance to not more than four individuals in a homelike setting. Individuals who live in Adult Family Homes may require supervision and assistance with personal cares such as, but not limited to bathing, shaving, and dressing. They usually don't require on-going medical treatment and some are even able to self-administer medications. Northwest Pathways To Independence, Inc. is an agency providing community based residential services for adult individuals with a cognitive or physical impairment. We currently provide services for 48 individuals in 15 settings primarily in the Chippewa Valley and surrounding areas. NPI is interested in opening 1-2 new homes for individuals in need. We are accepting referrals for individuals with cognitive impairment, traumatic brain injury, personal care needs, and other adults needing long term care.
